The Convalescent Care program helps people recovering from illness or injury return to independent living at home.
Following a hospital stay and after the acute phase of illness has passed, your care team may recommend convalescent care as the next step before you return home. Shalom Village provides the support and environment to help participants rebuild strength, reduce their need for assistance, and re-establish their independence.
What Convalescent Care Looks Like
The Shalom team works collaboratively with participants, their families and physicians to prepare them to manage their daily activities. You will wear your regular clothes and help make your own meals in the dining room. We offer a variety of recreational and rehabilitative activities, to enable you to be an active participant in your own recovery.
A Collaborative Approach to Recovery
The Convalescent Care program at Shalom Village is a partnership between Hamilton Health Sciences, Shalom Village, Dundurn Place Care Centre, St. Joseph’s Healthcare and Hamilton Community Care Access Centre.
The goal of the Convalescent Care program is to help participants regain strength, improve functioning and build confidence to ensure a smooth transition as you move back to your home from our home.
